Not exactly...

You miss a important point on your comment in the AI. But... was ok.

The name "file-name.ext" link title is the default page title for the 
file, but it is editable, without change the file name itself.

The people who see a download button (/or who listen "download" as a 
link name/) will not mistakenly click on the page view link if they 
wants to download.

I also disagree with what may be more useful for the blind guy, as i 
comment on the AI. Images means nothing for that guy.

Anyway, i like your argumentation pointing blind people issues!
