Currently, the `follow` method is called manually for every action that requires it (like adding members in a community). This MR adds the action creation to a filter in RoleAssignment, so there is no risk of forgetting to trigger the follow relationship.

It also adds a migration to create followers from RoleAssignments that for some reason did not create.
